1. Do you have a qualified high deductible health plan? (consult employer or insurance agent if unknown)
2. Do you have single or family coverage?
3. Do you have an existing HSA that you will be transferring to First Federal?

Application Process:

1. You can print the application, complete it, and bring it in to any of our offices, or the application can be completed in any office when opening an account.
2. Photo identification required to open an account (for new customers only).
